We use proven information security safeguards
We employ safeguards designed to protect the confidentiality of each patient’s medical records, patient accounts, clinical information from information systems, and confidential conversations. We secure all protected health information, utilize progressive technology and enforce regulatory mandates to limit access to medical information systems for patient privacy.
We limit the disclosure of patient information
We limit the disclosure of protected health information such as patient medical files to only authorized healthcare providers. An electronic audit trail follows each participating Sandlot healthcare provider, and Sandlot regularly reviews those electronic audit trails to ensure that only authorized healthcare providers are accessing your medical records. Furthermore, we do not disclose or use health information for marketing purposes.
We hold ourselves responsible for our privacy principles
All patient privacy practices and procedures are reviewed and approved by our Board of Directors, comprised of practicing physicians who are committed to providing quality medical care.
We also conduct internal and external assessments of our operational security and privacy practices. Sandlot commissions outside expert reviews annually of our compliance with the Security and Privacy Principles and the specific policies and practices that support these Principles. Sandlot’s annual security audit ensures Sandlot and its business partners adhere to the patient privacy standards.
We provide all Sandlot employees with training programs designed to educate our employees about the meaning and requirements of our patient privacy practices and procedures. Employees who violate these commitments are subject to disciplinary action up to and including dismissal and criminal and civil penalties. Employees are expected to report any violations to their manager or Security Officer.
We extend these privacy principles to our healthcare partners
We require companies we select as our healthcare partners, such as laboratory centers, radiology providers, and hospital systems, to agree to keep patient medical information confidential and secure. Our healthcare partners must also protect patient medical information against unauthorized access, use, or re-disclosure by the healthcare partner, and limit its use to the purposes for which it was accessed.
We ensure information quality
We use advanced technology, documented procedures and internal monitoring practices to ensure that patients are accurately matched to their medical data. We reliably identify the person for whom service or treatment is intended, and ensure that this information is accurately contained in the patient’s medical record.
