The Solution
Sandlot Connect
Sandlot Connect builds a comprehensive medical record, including diagnoses, medication lists and allergies. Our solution also allows medical document sharing, including imaging, lab results, physician letters and examination reports via an integration profile called XDS, or Cross-Enterprise Document Sharing. This software provides a complete patient medical record at the point-of-care, allowing physicians and nurses to view a comprehensive patient profile all in one place.
Patient data is collected from all connected data services to match patient records automatically, with robust connection source systems that expedite searches for patient information, and deliver higher data quality that is 100% compliant with integrating the health enterprise (IHE) standards. The result—reduced time for patient data clearing, lower administrative costs and improved patient safety by lowering the frequency of patient misidentification and medication errors.
Sandlot Dimensions
Sandlot Dimensions is a data repository that receives, analyzes and sorts clinical and claims information from a variety of sources in many forms and formats. It also stores disintegrated data so physicians can retrieve specific information to provide the highest quality, evidence-based care for their patients.
Sandlot Metrix
Sandlot Metrix prospectively tracks at-risk patients falling outside quality measures and delivers notifications at the point of care, within the physician’s electronic medical record workflow. Doctors are able to take immediate action, such as scheduling a diagnostic test, ordering a procedure, e-prescribing or ordering a laboratory test.
Additionally, Sandlot Metrix retrospectively monitors performance through the healthcare effectiveness data and information set (HEDIS), physician quality reporting systems (PQRS), meaningful use objectives, accountable care organization (ACO) requirements and other pay-for-performance measures. Sandlot Metrix has pre-built medical specialty patient measures that are important in evaluating patient health and diseases to allow physicians to efficiently determine which patients are not meeting specified measures of quality care. All of this works to increase patient satisfaction and enhances the physician’s ability to treat patients more completely at the point of care.
Sandlot Measure Builder is a tool within Sandlot Metrix that enables rules-builder functionality and is used to interpret and report on Centers for Medicare and Medicaid Services (CMS) and other required measures.
Sandlot Care Manager
Sandlot Care Manager reduces administrative costs and improves the efficiency of care coordination with patient and case relationship management workflow capabilities to provide the right care, at the right time, from the right resource. It features integrated workflow between caregiver roles and leverages both clinical and claims information streamlines the content for a rich view of case data. The result is near real-time data from integrated sources highlighting needed actions and interventions. Gaps in care, as well as recent clinical and stratification metrics, are made more visible. Care plans are managed using an efficient, prioritized and patient-specific task list.
Our Experience
Sandlot Solutions currently provides the health information exchange (HIE) and quality analytics technology infrastructure to support the one of the first Pioneer ACO’s in the country Model for North Texas Specialty Physicians (NTSP) and its more than 50,000 Medicare and Medicare Advantage patients. Established in 1995, NTSP is a physician-led multi-specialty practice has more than 600 family and specialty physicians who have been delivering high quality medical care to over 15,000 patients a day across 100 sites. Sandlot Solutions has also been recognized as a state-qualified Health Information Service Provider (HISP).
